Diabetes & High Blood Pressure Health Coach Assisted Lifestyle Program

We want to inform all individuals about a FREE 12-weeks lifestyle program for diabetes and high blood pressure. The program will focus on diet, exercise, stress, diabetes and high blood pressure management. The program will be held at the United Methodist Church located at the Mile ground, Morgantown; each participant will be assigned a trained health coach. Free glucose, cholesterol, and survey testing sessions will also be done to assess progress. This is a research project approved by the WVU Institutional Review Board.

You will become educated and empowered to manage both diabetes and high blood pressure, learn the easiest way to improve diet and physical activity without completely changing your life, learn how to manage your stress, and receive a $75.00 incentive upon completion of the program!

Eligibility Requirements include the following: be over 18 years of age, be diagnosed with diabetes, be diagnosed with high blood pressure, be willing to be randomized to receive the program immediately or after 6 months, be overweight. not be pregnant or breastfeeding, and have no physical activity limitations.
